Columbia has admitted to an authoring error on the Monty Python Holy Grail disk. You may contact them about a replacement at CTHV_Customer_Service@spe.sony.com

I recieved my replacement today. I have some good news and some bad news. The bad news is that the first Sweedish caption that appears below the films title is missing. Also, I can't remember if the faulty disc was like this, but you cannot switch between the various subtitles unless you go to the menu.

The good news is that the subtitle problem where they appear even though they are turned off has been fixed. I haven't watched the whole film yet, but I checked the scene in Chapter 1 where Arthur says, "Halt!" and they did not appear as they did before.

For reference. I am using a Toshiba SD-3108.
