How does a Tivo work with premium channels?

I'm thinking about making the jump to Tivo. My only concern is how/if it will work.

Idealistically, I'd like an HD Tivo, but since I can't get one (price and service) and since regular tivos are so cheap, i might just get a regular one until HD ones drop and are more available.

Right now I have a really nice cable package with HBO, Showtime, you know, the works. I have a CableCard plugged into the back of my Mitsubishi DLP which allows me to watch all that without a cable box.

Now, how will a Tivo work? Do I have to get a PVR from my cable company? Can I buy a Tivo and use my CableCard on it to decrypt the channels?

I just want to know how this works.

Hmmm, the card is in the TV itself? So it has an internal tuner. With a Tivo, the card would go into it. Call your provider to see if they have a version of Tivo to sell, or how their stuff would work with Tivo. Either way, premium channels are like any other channels, you order them, then you have them automatically, and Tivo can record from them.
