I'm not sure if it's the theatrical cut, but I have the Cleopatra 5-Star DVD and I believe it clocks in at 248 minutes.

JFK. I have the Director's cut but I know the theatrical is 3:18.
Godfather II might be a bit longer than that

The Lord of the Rings: The Two Towers which clocks in at 2:59 is my longest theatrical movie.

I do have thee others that are longer including Pearl Harbor (DC), LOTR: FOTR (EE), and LOTR: TTT (EE). Plus five TV shows (24 S1 & S2, Alias S1 & S2, and Band of Brothers) that are even longer yet.

Re: Longest movie in your collection? (theatrical version)

Longest:
Once Upon a Time in America: 227 min
Lawrence of Arabia: 227 min

180+ minutes:
War and Peace (Hepburn): 208 min
Giant: 201 min
The Godfather Part II: 200 min
The Right Stuff: 193 min

150+ minutes:
Camelot: 178 min
The Godfather: 175 min
The Sound of Music: 175 min
My Fair Lady: 173 min
Patton: 171 min
Scarface: 170 min
The Godfather Part III: 169 min
Saving Private Ryan: 169 min
Once Upon a Time in the West: 165 min
It's a Wonderful Life: 160 min
Gladiator: 155 min
Robin Hood: Prince of Thieves: 155 min
Pulp Fiction: 154 min
West Side Story: 152 min
T2: 152 min

Of course, I have a few "Director's Cuts" that are longer, but since you limited me to theatrical, this is all I've got. (I think )

Not sure about the Once upon a time in America, was the extended version ever released in theatres, or was it the 139 runtime?

Just at Cannes and the like, I believe. The trimmed version is what was released to the public. Very sad. It's a masterpiece.

My longest is Lawrence of Arabia.

Yes, Cleopatra is the roadshow theatrical cut

Maybe not the theatrical cut (was released in Russia as 4 films, and the US version was 360 minutes):

War and Peace - 431 minutes.

Theatrical Versions only means I must exclude LOTR all together, because I only own the EE's. So, my longest movie is:

Fiddler on the Roof = 179 mins = 2 Hours 59 Mins

Spartacus is the longest movie in my collection, but I'm not sure the version we have on DVD is the theatrical cut. Wasn't the Crassus/Antoninus bath scene left out during the original theatrical run? I know Anthony Hopkins had to dub Olivier's lines for the DVD release for whatever reason.
